1) What to do in preparation for your first visit:

  • Please fill out a health history form and bring it with you to your appointment.
  • Make a list of questions that you wish to ask the doctor.
  • Bring a list of your current medications or the prescription bottles.
  • Make sure your doctor has received a complete set of your medical records.

2) What to expect at your first visit:

  • During your first consultation, your health history form will be collected and reviewed. Your doctor, nurse practitioners, and/or resident fellow will focus on the details of your arrhythmia problems. It is important that you provide us with thorough information about your medical history.
  • Please be prepared to ask questions in order to best address any concerns you may have.
  • An ECG may be performed to evaluate your current heart rhythm.
  • At the end of your evaluation, the recommended treatment plan may include ordering additional tests and/or procedures to further evaluate your condition or assist in your treatment plan. Our patient coordinator will contact you by phone about scheduling these tests and/or procedures. Please allow a minimum of five working days to verify insurance and to obtain authorizations, if needed. Once you are financially cleared, the patient coordinator will serve as the liaison in the scheduling process.

3) What to bring with you:

  • Your completed health information forms
  • List of questions and/or concerns
  • List of your current medications (including dosage and frequency)
  • All contact information for your referring physician(s)

4) Billing:

  • The cost of your medical care will vary depending on your diagnostic and treatment needs. The consultation fee for your initial visit is $300-$400, and a follow-up visit ranges from $80-$120. The fee for your visit will be for physician services only and does not include laboratory work, x-rays and other tests. As a courtesy, we will bill your insurance company; however, you will be responsible for all co-pays at the time of service. You will also be responsible for all charges not paid by your insurance carrier. If you do not have insurance, please be prepared to pay at the time of your visit. The doctor's fee for your visit will be billed in two separate statements. One portion of the professional fee will be billed under UCLA Medical Group and the other portion will be billed by Ronald Reagan UCLA Medical Center as a "facility fee".

5) If you require a medical translator:  

  • Please bring a family member or friend who can assist.
  • Please call (310) 825-7636 in advance to make appropriate arrangements with the UCLA translation services.
